American Heritage Life Insurance Company (“Allstate Benefits”) Terms and Conditions for Electronic Transactions and Electronic Delivery

These Terms and Conditions for Electronic Transactions and Electronic Delivery (“Terms and Conditions”) apply to forms, disclosures, notices or other documents that may be given to you, and in some cases signed and returned to us, as part of an insurance transaction or claim. To transact business electronically with us, as well as receive your insurance related documents electronically (“E-Delivery”), you must read and agree to these Terms and Conditions. By agreeing to these Terms and Conditions, you are agreeing you have the authority to receive such documents electronically and acknowledge you can electronically access, view, print and save those documents.

The words “we,” “us,” and “our” means Allstate Benefits, its affiliates, or parent company. The words “you” and “your” means you the individual who desires to transact business electronically.

By enrolling in E-Delivery and using Allstate Benefits websites, you agree that Allstate Benefits may discontinue sending paper documents and provide any insurance related documents to you in electronic form, and you are indicating your desire to conduct business electronically with Allstate Benefits in the future unless you withdraw your consent or we unenroll you from E-Delivery. Your consent to E-Delivery applies to all insurance products purchased by you from us.

1. Method of Providing Documents to You in Electronic Form. All documents that we provide to you in electronic form will be provided either (1) via e-mail; or (2) by your accessing an Allstate Benefits website. You will need the username and password that you created to access the website if using an Allstate Benefits website to access information.

2. How to Withdraw Consent. You may unenroll from E-Delivery by indicating your preference on the Profile page or similar page in your online account on the applicable Allstate Benefits website. You may withdraw your consent to transact business electronically and unenroll from E-Delivery by contacting us at 1-800-521-3535. At that time, your online account will be terminated. We will not impose any fee to process the withdrawal of your consent to transact business electronically. If, however a discount is given now or in the future for enrolling in E-Delivery, a withdrawal of consent could result in the removal of any such discount. A withdrawal of your consent to transact business electronically will be effective only after we have had a reasonable period of time to process your request.

3. How to Update Your Records. It is your responsibility to provide us with an accurate and complete e-mail address, and to maintain and promptly update any changes to your contact information. You can update your contact information on your Profile page or similar page in your online account on the applicable Allstate Benefits website, by contacting us at 1-800-521-3535 or by writing to us at Allstate Benefits, 1776 American Heritage Life Drive, Jacksonville, FL 32224. If you provide an invalid email address, or a previously valid email address malfunctions or becomes invalid, you will be automatically unenrolled from E-Delivery when we actually become aware of such invalidity.

4. Hardware and Software Requirements. In order to access and view electronic documents that we make available to you, we recommend you:

  • a. Have an electronic device with internet connectivity, a valid Internet service provider and one of the following:

    i. For Windows Operating System, Internet Explorer 11 or higher, Google Chrome 60 or higher, Firefox 55.0.3.0 or higher, Safari 5.1.7 or higher; or

    ii. For Macintosh Operating System, Safari 11 or higher.

  • Other browser types or versions may result in an inconsistent or poor experience.
  • b. Have the ability to open PDF documents on your electronic device.
  • c. Have sufficient electronic storage capacity on your computer's hard drive or other data storage unit.

Browser settings may impact the user experience, particularly with mobile devices. Settings such as having cookies enabled, or being in private browsing mode, can impact your ability to access Allstate Benefits websites. We will update these Terms and Conditions if there are any changes to the hardware or software requirements that could impact your receiving documents electronically.

5. Requesting Paper Copies. You can obtain a paper copy of any document we provide to you electronically by printing it yourself or by requesting that we mail you a paper copy by calling us at 1- 800-521-3535. There is no charge associated with requesting a paper copy of a document we sent you electronically. You agree that requesting a paper copy of a document provided to you electronically shall not invalidate your E-Delivery election. We reserve the right, but assume no obligation, to provide a paper (instead of electronic) copy of any document that you have authorized us to provide electronically.

6. Mandatory Paper Copies of Electronic Communications. We will not send you a paper copy of electronic documents unless you request it, we otherwise deem it appropriate to do so for any reason, or in some instances where we are required by law to send paper copies of documents. While you may elect to receive documents electronically from us, there may be documents that we need to provide to you in paper form. Your prior E-Delivery election shall not be invalidated when mandatory paper copies are provided.

7. Communications in Writing. All documents in either electronic or paper format from us to you will be considered “in writing.” You should print or download for your records a copy of all electronic documents, and these Terms and Conditions.

8. Law. You acknowledge and agree that your consent to transact business electronically is being provided in connection with a transaction affecting interstate commerce that is subject to the federal Electronic Signatures in Global and National Commerce Act and your state’s UETA law.

9. Termination and Changes. We reserve the right, in our sole discretion, to discontinue the provision of your electronic documents, or to terminate or change the terms and conditions on which we provide electronic documents or transact business electronically. We will provide notice of any such termination or change as required by law.

10. Confirmation. You will receive a confirmation email from us once you consent to transacting business electronically with us. You should contact us if you do not receive this confirmation email within 5 business days.

E-delivery services are provided on an “AS-IS” basis. Allstate Benefits does not guarantee that these services will be without defect or flaw and Allstate Benefits will not be held responsible for any loss associated with the use of these services.

By agreeing to these terms and conditions, you are confirming that you are able to receive the documents pursuant to the hardware and software requirements noted above and indicating your desire to conduct business with us electronically until such time as you withdraw your consent.
