How to list your U.S. Social Security Number (SSN) on your passport application, and what to do if you do not yet have an SSN, or cannot remember your SSN.
I do have a U.S. Social Security Number.
If you were ever issued a Social Security Number, you must provide it at item 5 on your application for a U.S. passport. If you fail to provide the information, you will encounter a delay in processing and/or denial of your passport application. You will also be subject to a $500 penalty enforced by the IRS pursuant to Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E). All questions on this matter should be directed to the nearest IRS office.
It is advisable to bring:
- your original Social Security Number card, or
- a photo copy of it, or
- have a scan of it available on your phone.
I do not have a U.S. Social Security Number.
If you do not have a Social Security Number, you must submit a statement, signed and dated, that includes the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security number by the Social Security Administration.” Sample of a Social Security Number declaration over age 16 (PDF file 88 KB)
For a minor child that does not have Social Security Number, a parent must submit a statement, signed and dated that includes the phrase, “I (name of parent), parent/guardian of (name of child applying) declare under penalty of perjury under the laws of the United States of America that the following is true and correct: My child has never been issued a Social Security number by the Social Security Administration.” Sample of a Social Security Number declaration under age 16 (PDF file 166 KB)
On your application form DS-82 or DS-11 you must enter zeros at item no. 5.
Upon receipt of your new passport suggest you apply for a Social Security Number as soon as possible. The application begins by contacting the Federal Benefits Unit in Dublin, Ireland.
